Has the issue to do this ACS20001, ACS50000 and ACS50033 been fixed? I've tried it over the last couple of days but I get the same error.

Most of the illustrations have been replaced with i2.asp.net/ which is a bit unhelpful.

Also, can you let me know whether this is supposed to work with Visual Studio Express 2012 for Web? I definitely have the fall 2012 update installed but there's no "Enable Windows Azure Authentication" option on the project menu.

You are missing Microsoft ASP.NET Tools for Windows Azure Active Directory – Visual Studio Express 2012 for Web - you need to install that to get the "Enable Windows Azure Authentication" option on the project menu.
